Four-tier prescription copay structure through Rx Choice empowers members to manage costs without sacrificing choice — including Costco®, CVS Pharmacy®, Kroger®, Meijer®, Sam’s Club®, Target®, Walgreens®, and Walmart®.
EnsureRx lets employees compare costs of generic and brand-name medications and find discounts on hundreds of covered and noncovered drugs.
CarelonRx Pharmacy Home Delivery allows employees to save time and money on 90-day supplies of maintenance medications, including many common and select generic drugs at no extra cost.7
CarelonRx Specialty Cost Relief provides $0 copay for specialty drugs and innovative therapies used for a variety of health conditions.8

8 Employees must enroll in the program, and cost share could vary depending on member enrollment. Employees who do not wish to participate in this program can opt out and will be responsible for the full amount of the cost share for their specialty drug. Employees who have a high-deductible health plan with a health savings account are required to meet their deductible before they can receive a $0 copay. Certain cost relief drugs are not classified as “essential” under the program and in accordance with the Affordable Care Act for these cost-relief drugs. Claims for these cost-relief drugs would not be subject to a member’s out-of-pocket limit. Client should consult legal advisor in considering this benefit feature.

The SmartShopper program is offered by MDX Medical, LLC, a Zelis company. Reward-eligible options and reward amounts are subject to change. Rewards are available for select procedures only. Rewards may be a taxable form of income. MDX Medical, LLC, a Zelis company, does not provide tax advice. Rewards may be delivered by check or an alternative form of payment. Members with primary coverage under Medicaid or Medicare are not eligible to receive incentive rewards under the SmartShopper program.
In addition to using a telehealth service, members can receive in-person or virtual care from their own doctor or another care provider in their plan’s network. If they receive care from a doctor or care provider not in their plan’s network, their share of the costs may be higher. They also may receive a bill for any charges not covered by their health plan.
Sydney Health is offered through an arrangement with Carelon Digital Platforms, a separate company offering mobile application services on behalf of your health plan.
