Caregiving is an act of love, but it’s work, too.
If you’re caring for someone who needs help with daily living activities, such as feeding and bathing, they may be eligible for Indiana PathWays for Aging (PathWays)— a Medicaid plan for older Hoosiers that makes living at home possible.
PathWays is a managed long-term services and support program
PathWays is an Indiana health coverage program for Hoosiers aged 60 and older who are eligible for Medicaid. Research shows that most older adults — 75% or more — want to age at home and in their communities. PathWays makes it possible for older Hoosiers to have a choice in how and where they age. A nursing home might be the right choice for some individuals. PathWays offers more choices that allow individuals to get nursing facility level of care at home or in a community setting. PathWays helps older Hoosiers live as independently as possible for as long as possible.
PathWays is designed to help members access the healthcare and community-based services they need and want. As a person-centered health plan, our goal is to focus on what matters most to our members and involve them in all decisions about their health. We support our members to live where they choose. We’ll coordinate their care between doctors, specialists, caregivers, and in-home services. And we’ll ensure they have access to the services and support they need for the best quality of life.
Who is eligible?
Your loved one may qualify if they are at least 60 years of age and have limited income and resources. PathWays members includes those who are:
- Blind or disabled
- Getting Supplemental Security Income (SSI)
Enrolled through M.E.D. Works
- Full-benefit dually-eligible individuals (have both Medicare and Medicaid)
- Eligible for the Aged and Disabled (A&D) waiver
- Living in a nursing facility
Receiving hospice services
Individuals with Medicare
With PathWays, we will work with your loved one’s Medicare providers and care managers to ensure seamless care. If your loved one is not yet on Medicare, or if you have questions about how Medicare and Medicaid work together, we can help.

Our members choose their path of care
We work hand in hand with members and caregivers to understand what each member wants and needs — whether it is specialty care, stable housing, or comfort from loneliness. We will ensure that members get the best possible care, support to make their lives easier, healthier, and have more choice about how and where they spend their days.
Our care and service coordinators will make sure that our members' voices are always primary in the planning process, while facilitating shared decision-making. We will assure that each member, supported by their caregiver, understands their options, based on their needs, and that services are delivered seamlessly in the setting they choose.
