What you get with the Anthem HealthKeepers Plus plan

Get the care you need from a doctor you choose. We have thousands of doctors in our plan, including many specialists.
We pay for prescriptions and many over-the-counter medicines. You can get your medicine from any pharmacy in our plan.
When you have to get care in the hospital, you won’t have to pay anything. Benefits also include urgent and emergency care.

Healthy teeth are important for both kids and adults. All members can now get dental benefits, including exams, cleanings and X-rays.

Vision benefits for members 21 and older include an eye exam every other year and up to $150 for glasses or contacts yearly. Members under 21: glasses or contacts as medically necessary, plus routine eye exam.

  • Baby Essentials catalog – select up to three items from the Baby Essentials catalog after completing prenatal and postnatal exams
  • Books for Kids program – $35 Barnes & Noble gift card for children ages 10 and under
  • Good grades program – $25 gift card for eligible students in middle school to college (Students must receive all As and Bs over one grading period in the current school year.)
  • Chromebook for qualifying high school seniors with a GPA of 3.5 or higher in the current school year
  • Yearly sports physical for members ages 19 and under
  • Yearly Boys & Girls Club membership at participating locations for members ages 17 and under

New to Medicaid?

Gather up important information for you and any family members who are applying for health care coverage:

Names, birth dates and Social Security numbers

Most recent federal tax information  (if available)

Pay stubs or other job and income information for the last month

Other taxable income like unemployment benefits, Social Security benefits, pensions, retirement income, rental income, alimony received, etc.

Apply in one of these ways:

Monday through Friday from 8 a.m. to 7 p.m. and Saturday from 9 a.m. to 12 p.m.

Call or visit your local Department of Social Services office

Once you’re approved, you’ll be assigned to a health plan. You’ll have 90 days to switch to a different plan after you start.

You can switch plans during the first 90 days of enrollment and then each year during open enrollment. Open enrollment dates depend on where you live. Find your open enrollment period
