What you get with the
Anthem HealthKeepers Plus plan

Get the care you need from a doctor you choose. We have thousands of doctors in our plan, including many specialists.
We pay for prescriptions and many over-the-counter medicines. You can get your medicine from any pharmacy in our plan.
When you have to get care in the hospital, you won’t have to pay anything. Benefits also include urgent and emergency care.

Healthy teeth are important for both kids and adults. All members can now get dental benefits, including exams, cleanings, and X-rays.

* Dental care is offered by the Department of Medical Assistance Services.

Vision benefits for members 21 and older include an eye exam every other year and up to $150 for glasses or contacts yearly. For members under 21, you can get: glasses or contacts as medically necessary, plus routine eye exam.
Health issues come up when you least expect them. Get fast, free medical advice from a registered nurse anytime, day or night.

With the Anthem HealthKeepers Plus plan, you may be eligible for these extra benefits:
 

Extras for kids

  • $35 gift card for books for children ages 10 and under.
  • Yearly sports physicals for members, ages 11–19, who are currently enrolled in school.
  • $100 gift card for members ages 5–17 to use toward annual membership fees to youth clubs such as: Boys & Girls Club, Boy Scouts, Girl Scouts, 4-H Club.
  • Receive a $100 Healthy Grocery gift card to purchase nutritious groceries, for members ages 5–17 diagnosed with obesity or diabetes.

Extras for pregnant moms

  • Up to two $150 gift cards to purchase essential pregnancy and baby items after completing prenatal and postpartum visits.
  • Up to four $100 Healthy Grocery cards for pregnant moms.
  • A $100 gift card to purchase items that support healthy breastfeeding, such as: breast pump accessories, disposable nursing pads, extra milk storage bags, bottles, microwave sterilizer.
  • Up to four $100 gift cards to purchase over-the-counter products.
  • Select a $100 gift card for one of the following transportation options: a rideshare card, a gas card, or for a public transportation pass.

Extras to live well

  • Access to the Community Resource Link to find all the available local community-based programs, benefits, and services.
  • Eligible non-pregnant female members, age 8–26 can receive up to four $100 gift cards to purchase feminine.
  • Select items from the Healthy Lifestyle Aids catalog such as: digital scale, home blood pressure cuff, diabetic insoles and socks, digital thermometer, pulse oximeter, heart rate monitor, abdominal binder, lumbar support items, activity tracker, memory foam pillow.
  • Choose one item from the Asthma and COPD Relief catalog, featuring relief solutions such as: inhalers, nebulizers, hypoallergenic bedding, pillow and mattress covers, and HEPA air filters — available to eligible members.
  • Adult vision benefit — Members age 21 and over receive one eye exam yearly. Plus, up to $250 toward the purchase of one pair of eyeglasses or contacts per 12 months.
  • Community transportation — Members can receive up to 24 round trips annually to go to the grocery store, food bank, job interviews, places of worship, government offices (DSS, DMV, post office, and Social Security office), and support groups.
  • Transportation essentials — Eligible members can select one of the following annually:
    • $100 gift card to purchase public transportation pass
    • $100 rideshare card
    • $100 gas card
  • $20 card for completing MMHS Screener.
  • Hearing aids — Up to $2,000 per year for hearing aids and a year’s supply of batteries in common sizes.
  • Unlimited access to the online Emotional Well-being program for members age 13 and older on their journey to become and stay mentally and physically healthy.
  • Yearly subscription to a meditation app for members diagnosed with a behavioral health condition.
  • Online SUD Recovery Support Program for members age 13+ with substance abuse, can receive access to a mobile platform that provides daily motivation, peer support groups, peer to peer messaging, counselor messaging and more.
  • Eligible members can select two items from a home safety catalog, such as: carbon monoxide detectors, medication lock boxes, smoke alarms, fire extinguishers, and more.
  • $75 gift card to purchase sensory products for members diagnosed with ADD/ADHD, anxiety, or autism.
  • Receive home-delivered meals after a 48-hour or longer inpatient stay.
  • Eligible members can receive a digital three-month voucher for the WW® core membership through the WW® app.

 Extras to take the next step

  • Members ages 16–21 pursuing a post-secondary education within the next year can select one of the following options:
    • $50 gift card to purchase SAT/ACT study guides.
    • $100 gift card to use toward college application fees.
    • $100 gift card to use toward textbooks and dorm room items.
  • Members enrolled in the Job Connections Program can select one of the following options to help support their employment pursuits:
    • Industry certification assistance — $100 gift card to help cover the costs of certifications.
    • Uniform support — $100 gift card to purchase employment required uniforms or work attire.
    • Styling support — $100 gift card to use toward supplies or grooming products.
    • Tools purchase assistance — $100 gift card to purchase work tools.
  • Smartphone with preloaded digital health applications and resources to help quickly access care and stay healthy; Case Management referral required.
  • GED testing assistance to help cover the cost.
  • Members age 15+, can receive $35 gift card to help cover the cost of obtaining a driver’s license, birth certificate, Social Security card, or state identification card.

 Extras for the Foster Care Specialty Plan

  • A Chromebook when members age 17 and older complete a Health Risk Assessment (HRA).
  • Members up to age 18 can receive a $100 gift card to purchase clothing.
  • Earn a $25 gift card for achieving a GPA of 2.5 or higher during one semester in the current school year. Available to members ages 11–18.
  • Members can choose one package to help promote family togetherness.
    • Family Game Night — includes a card game, jumbo playing dice, jigsaw puzzle, classic board game, and a $50 restaurant gift card.
    • Family Movie Night — includes $50 movie gift card and $50 restaurant gift card.
  • Members age 18+ can receive a 24-inch suitcase.
  • $50 gift card to enroll in various online classes including but not limited to nutrition, cooking, and fitness related courses for members ages 6–18.
  • Members can receive up to four $25 cards yearly to purchase personal care and hygiene products.

New to Medicaid?

Gather up important information for you and any family members who are applying for healthcare coverage:

Names, birth dates, and Social Security numbers

Most recent federal tax information  (if available)

Pay stubs or other job and income information for the last month

Other taxable income like unemployment benefits, Social Security benefits, pensions, retirement income, rental income, alimony received, etc.

Once you’re approved, you’ll be assigned to a health plan. You’ll have 90 days to switch to a different plan after you start.
